This was on ZD 1.10beta
I was playing with Bless on a map in a wad called Jacobs Ladder.
We both had this happen to us at the same time, we walked over a BFG while we had the SSG out, and both of our shots on one another ended up being blank. It only seems to happen when you walk over a weapon and try firing after pickup.

I didn't find any settings that could have affected this serverside.
On my client, I use PWO and my SSG is set to slot 1. I do notice that even with PWO being active, sometimes it seems to always want to make me change weapons on pickup. This might possibly factor into the problem.
I can try to reproduce it and see if I can get more info.

My belief to this bug was inconsistency in the emulation of safety shot mechanism for weapons such as the Rocket Launcher and the BFG9000.
The idea of this mechanism is to make it impossible for the player to fire right away upon switching to either of these guns in order to prevent blasting away yourself upon picking the RL, or waste 40 cells upon using the BFG.
I have just conducted a test on ZDaemon 1.10.07 on a local server and couldn't reproduce it. Tested in the following order:
In all of the cases, I was able to hold the trigger, pickup a new weapon, and fire right away without any noticeable outsync, which raises another question: Should this feature be emulated at all?
As far as the bug report goes, I could only spot the following entry in the changelog:
